It is always better to have travel insurance when you go on a trip as it can be very expensive to pay for medical expenses from your own pocket. In some cases, you must have insurance - a few countries in Europe, Canada, the United States of America, Australia, and New Zealand do not issue you a visa unless you have medical, which comes bundled in your travel insurance.
