It is always recommended that you buy a travel policy for the full duration, such as a year or six months, instead for the supposed duration of the trip. Extensions on your travel insurance policy depend on the terms and conditions of the insurance company, and you might not get the extension.
In case you decide to shorten your stay before the policy expires, you can reimburse the premium for unused policy duration (where the minimum unused period must be 30 days). For instance, if you purchase a policy with an initial duration of six months but decide to come back in four months, you can get the premium for the unused policy duration reimbursed.
